How realistic? Check them out. Have 3D eyes and a realistic scale finish. With these baits you won't ever have to use live bait again (well maybe).

To bad as far as I know are not available here (but these days anything is possible). Where are they made? Poland.

Is there a big difference in the paddle tail being upside down compared to most others? All the ones I see in the US are flipped around interesting

You don’t get as much lift with it upside down, but they are mechanically the same. All paddle tails get their action by rolling the tail section of the bait on its axis. Body wobble and head movement are both a function of how much the tail section allows the paddle to twist the entire bait.
